Automated data exchange
SFDPH shares information like discharge summaries, visit notes, test results, and imaging reports for most patients through health information exchange (HIE). We participate in the following HIEs:
If you have a certified electronic health record (EHR), look for this information through your EHR's health information exchange viewer. Please work with your own organization’s IT Department to learn how to connect with SFDPH via HIE, use your EHR's health information exchange viewer, or to set up receipt of outside event notifications.

Provider portal
With SFDPH CareLink, community users like referring and ordering providers, post-discharge facility staff, and high-volume release of information requesters can access the patient chart via a web portal. They can:
- follow a patient's care across the health system
- schedule appointments
- place orders
- receive notifications
- send notes
- and more
Option above requires a data sharing agreement.
