If you receive public benefits, you may need to show proof. For example, when you apply for an immigration benefit, like citizenship.

What to do

Gather your information

To request your public benefits verification letter, you will need to provide: 

  • your name, 
  • address,
  • date of birth,
  • and the types of benefits you need to verify in your letter.

If you want a paper copy of your letter, you need to ask for it when you make contact with the Human Services Agency. 

Call or email to request your letter

You can call 1-855-355-5757 to request your letter.

You can also email the Human Services Agency at hsabenefitsverification@sfgov.org.

Public benefits verification letters are from the Human Services Agency (HSA). They are confidential paper records, mailed only to the direct recipients of benefits.

Special cases

Helping someone else

You can help someone else get their benefits verification letter. To do this, you must attach proof of consent from that person in the request to the email.

What happens next

After you send your email request, HSA reviews and researches the request to confirm an active case exists.

After an active case has been confirmed, a benefits verification letter will be mailed to the address on file that is associated with the case. This process can take 3- 5 business days.
