1. Learn about the program
Before you file a report, learn about the Whistleblower Program.
Be sure to review:
- How it works
- Your rights (including confidentiality and protection from retaliation)
2. Submit your report
On your report form, tell us:
- Your contact info (unless you would like to remain anonymous).
- Full and complete complaint details, explaining who, what, where, when and why.
- If you have reported this information to another venue.
Include any documents (photos, copies of emails) to support your report.

Tell the operator that you wish to file a report with the Controller's Whistleblower Program.

Check your report status
After you file a report, you'll get a complaint tracking number.
- Use your tracking number to check your report status here for reports filed before
July 1, 2024 - Use your tracking number to check your report status here for reports filed on or
after July 1, 2024.
When you check your report status, you will:
- Get a general update on your report status
- See any investigator messages if they have follow-up questions
Under current law, we do not disclose Whistleblower report information.
