For all complaints, tell us:

About you

  • Your name
  • Your contact information

About the incident

  • Date, time, and location of the incident
  • Name of the employee, if you have it
  • Name of the witness, if you have it
  • Description of the incident 

The complaint form is also available in Chinese, Spanish, and Tagalog.

You can also file a complaint by phone, mail, or in person.

If you want to make an anonymous complaint, you don't need to put your name or contact information on the form. But an anonymous complaint may be more difficult to investigate.

Next step

We will review your complaint and decide how to proceed. We conduct all investigations objectively, with a goal of maintaining public confidence and Departmental integrity.

Our department will contact you to let you know that we have received your complaint, and will notify you within 30 days of the disposition.

Documentation of the complaint and any investigation will be retained for 5 years.

Formal investigation

If we need a formal investigation, it is usually completed within 6 months. In some special cases, such as legal proceedings, it may take longer.
