The San Francisco Office of Labor Standards Enforcement (OLSE) advances the welfare of San Francisco’s workers through the enforcement of labor laws, proactive public education, and high quality public service.

Submit an Employer Annual Reporting Form to OLSE

Employers covered under by the Health Care Security Ordinance and/or Fair Chance Ordinance are required to submit the Employer Annual Reporting Form to OLSE each year. The 2022 Annual Reporting Form (ARF) is due on May 1, 2023.
The link to the form is no longer active, and the form can no longer be submitted. Employers who failed to file an Annual Reporting Form should retain records of their compliance with both labor laws.
