About Laguna Honda

Laguna Honda Hospital and Rehabilitation Center is a general acute care hospital providing medical, rehabilitation, and skilled nursing services. Located west of Twin Peaks, Laguna Honda has a rich history of caring for San Franciscans, dating back to 1866, and a strong community of patients and residents, families, and staff.

Community Living

Laguna Honda is organized into 13 specialized neighborhoods, or units, made up of approximately 60 residents, all of whom are likely participating in a similar care program. The neighborhood model provides a close-knit community. It also means dedicated and consistent staff.

Medical Clinics

Laguna Honda's outpatient clinics offer more than 20 medical and surgical specialties, with dental, podiatry, and optometry being the most frequently used services. The clinics are primarily staffed by consultants from the University of California San Francisco (UCSF). The convenience of our on-site outpatient clinics allows residents to receive many specialty services without having to leave the campus.

Rehabilitation

The goals of rehab include maximizing the ability to perform tasks of daily living (e.g. personal care, continence training), mobility (wheelchair use and/or ambulation), vocational rehabilitation, and other self-management tasks (medication management, wound care). We offer acute rehabilitation, which is intensive therapy for residents who have the potential for rapid functional gains, and skilled nursing rehab for those who require focused rehabilitation therapy (e.g., predominantly physical therapy needs).

Memory Care

Our memory and cementia care provides services for residents living with moderate to severe cognitive impairment. The program promotes resident choice, includes structured activities, and maximizes resident abilities in the most independent setting available to fit their needs. These include social environments such as the therapeutic farm and garden, art classes, and special hospital-wide entertainment and events.

Spanish and Chinese Units

Laguna Honda’s mission is to provide culturally competent healthcare to the diverse communities of San Francisco. Speaking with residents in their native tongue removes barriers to proper care and provides a welcoming and familiar environment. Enjoying specific foods, holidays, manners, and other cultural norms helps residents feel comfortable and at home.

Positive Care

Our dedicated HIV/AIDS neighborhoods was started over 26 years ago and continues to provide innovative care for residents with HIV/AIDS requiring skilled nursing services. Physicians are HIV specialists with many years of experience. We offer short term stays for rehabilitation, treatment, and/or stabilization of acute medical problems, complex medication management, and longer term stays for more debilitating medical issues or significant AIDS-related dementia. Additionally, the program offers respite stays for caregiver support and collaborates with the Palliative Care Program to enhance end of life care.

Palliative Care

Our palliative care program has three primary goals: clarify hopes, values and goals of care for residents, provide comprehensive symptom management for physical and psychological symptoms and spiritual distress, and offer bereavement support to resident families after death. Our interdisciplinary care team includes board certified palliative care professionals, a chaplain, and volunteers from Zen Caregiving Project. Services include: Spiritual Care Services, Pain and Healing Clinic, Palliative Care Specialist consultation, and Bioethics consultation. We operate in partnership with the Zen End of life Project.

Respite Care

The emotional and physical demands of caregiving can make it hard to be a caregiver 24/7. Respite Care Services are available to families caring for seniors or adults with disabilities at home. We offer families a temporary break from the responsibilities of caregiving, which can be a vital solution to ensure caregivers take care of themselves and continue providing the best possible care to their loved ones. It also allows caregivers to take vacations or time off to attend to their own health needs. Respite Care Services can be planned for up to one month stays and is covered by Medi-Cal as long as skilled level of care criteria are met.
