Collateral request form

The San Francisco Department of Emergency Management (SF DEM) provides printed disaster preparedness materials upon request to San Francisco residents, businesses, neighborhood groups, professional associations, non-profits, and other entities. SF DEM cannot fulfill every request for resources that we receive. For more information on getting prepared for emergencies, please visit sf72.org

Types of collateral you can request

  • Printed SF72 disaster preparedness materials about making a plan, and organizing disaster supplies
  • Printed resources about when to call 911 vs 311
  • Printed resources about signing up to receive emergency alerts from AlertSF
  • Printed resources for school-aged children
    • please specify age range/grade level and whether there is a particular topic of interest by checking "other" on the form

SF DEM resources are available in English, Spanish, Tagalog, Chinese, Russian
