What to do
If you are a subcontractor working under another subcontractor, file a Preliminary Stop Notice with the primary contractor before completing the following steps.
1. Fill out the form
Download the Stop Notice Lien form and fill it out.
On the form, tell us:
- The primary contractor's name and contact information
- The contract or job number
- The department name and number
- What work you performed
- How much money you are requesting
2. Mail in your completed form
