  • Pediatric Primary Care: birth to 24 years, but will only accept new patients up to 17 years old
  • Pediatric Urgent Care: age 18 and under

Pediatric Primary Care Services

Pediatric Primary Care Services

In addition to full scope services with a medical provider, our primary care patients have access to:

  • vaccination appointments
  • lab appointments
  • behavioral health services
  • navigation help with social needs (like food, housing and employment)
  •  breastfeeding support
  • developmental assessment
  • nutritional services.

Specialized Children's Health Services

Specialized Children's Health Services

Other specialized children's health services available by appointment or with a referral from the patient's primary care provider include:

  • newborn clinic
  • asthma/allergy
  • cardiology
  • dermatology
  • developmental pediatrics
  • neurology
  • obesity/healthy lifestyles
  • special needs

Our clinic also provides:

  • mental health evaluations
  • referrals
  • management including conditions such as depression and attention deficit hyperactivity disorder (ADHD)

Other specialties may be available at ZSFG in partnership with our adult medical colleagues.

HealthySteps and Health Advocates

HealthySteps and Health Advocates

Our clinic is a HealthySteps site. This means patients from birth through age five can get extra support to help their family thrive. The HealthySteps team gives extra help around:

  • parenting questions
  • connecting with community partners
  • supporting parents around their own health and basic needs 

Primary Care patients of all ages at the CHC can be connected to the Health Advocates. The Health Advocates team connects families with community programs that assist with needs like:

  • not having enough food
  • housing conditions
  • needing employment
  • other financial help.

The Health Advocates check in with families after a visit via phone to ensure the successful connection to the resources they need.

Transitioning to a new place and culture can be challenging. 6M Bridges Clinic offers support services to help newcomers (regardless of insurance or documentation status) feel at home in San Francisco. We aim to promote your family's health and well-being by working with our unique team of Family Navigators to connect you with dental care, mental health, legal and other community resources for your family.
