LEGACY provides peer-based support for families and transitional-age youth involved in San Francisco's child-serving systems. LEGACY promotes family and youth voices and supports the development of strong relationships among individuals, families, and service providers.
We serve individuals who have low incomes, are uninsured, or have/qualify for Medicare, MediCal, or San Francisco Health Plan.
Services include:
- Case Management (assessing needs, coordinating care, and connecting to other services)
- Assistance with systems navigation (Special Education, Behavioral Health, Child Welfare, Juvenile Justice) and advocacy
- Support groups
- Parenting classes
- Educational and life skills workshops
- Referrals to health and community resources (therapy, childcare, public benefits, and housing.)
Our team speaks English, Spanish, and Cantonese. Other languages are available via an interpreter service.
