Gender Health SF: Working with a peer patient navigator

After your doctor's referral, you'll be connected with a peer who can support you through the patient process.

About peer navigators

When you are referred to Gender Health SF for gender-affirming surgery, you will be connected to a peer patient navigator.

Your peer navigator will help you move through the process of getting gender-affirming surgeries and procedures. This includes:

  • Resources and information to help you reach your goals
  • Group and 1-on-1 education sessions
  • Choosing a gender-affirming surgeon
  • Helping you advocate for yourself
  • Coming with you to your appointments (as needed or desired)
  • Connecting you with groups led by your peers for topics like surgery education and aftercare
  • Wellness programs like quitting smoking, diet and nutrition, and mental wellbeing

We will be there to support you from the time of referral through your post-op surgical care and follow-up experience.

Who can get care

You can get patient navigation help if you get health care through the:

  • San Francisco Health Network
  • San Francisco Department of Public Health Clinic, or
  • Department of Public Health consortium clinic

We support access to care if you are under-resourced, uninsured, and underinsured.

You need a referral from your primary care clinic to get services with Gender Health SF.
