About peer navigators
When you are referred to Gender Health SF for gender-affirming surgery, you will be connected to a peer patient navigator.
Your peer navigator will help you move through the process of getting gender-affirming surgeries and procedures. This includes:
- Resources and information to help you reach your goals
- Group and 1-on-1 education sessions
- Choosing a gender-affirming surgeon
- Helping you advocate for yourself
- Coming with you to your appointments (as needed or desired)
- Connecting you with groups led by your peers for topics like surgery education and aftercare
- Wellness programs like quitting smoking, diet and nutrition, and mental wellbeing
We will be there to support you from the time of referral through your post-op surgical care and follow-up experience.
Who can get care
You can get patient navigation help if you get health care through the:
- San Francisco Health Network
- San Francisco Department of Public Health Clinic, or
- Department of Public Health consortium clinic
We support access to care if you are under-resourced, uninsured, and underinsured.
