Birth to Five Public Health Nurse Home Visiting Program
The Public Health Nurse (PHN) will provide home visits within the City and County of San Francisco to children ages birth to five with special health care needs in in-home and out-of-home placements. The goal of the program is to support families in assisting children to reach their full potential while maintaining optimal health in a safe and nurturing home environment. The desired outcome is to improve coordination of services, promote health, enhance development, and prevent disability.
Support for those caring for young children: Birth to Five Program
Caregivers of children ages birth to 5 may be able to get help from our team of nurses and partners. There is no cost for our services.
To qualify, the child must have a case in SF Family and Children's Services (CPS). There are services available to different types of caregivers.
Parents of children ages birth to 5 years
A public health nurse from our team can visit you and your child at home if you
- have questions about your child's health needs
- need help scheduling appointments
- would like support with parenting
- are worried about your child's development
Resource (foster) parents and relatives caring for children ages birth to 3 years
Our Infant Training Program provides guidance and information to caregivers of infants and children with complex medical needs and developmental challenges, including in-utero substance exposed newborns
Monthly Resource Family Support Groups in English and Spanish
Parents in SF Family Treatment Court
A public health nurse is a part of your family treatment court team and can
- visit you and your child at home or in your treatment program
- visit your child if they are out of home
- help you understand your child's health and developmental needs
- connect you to a SafeCare parenting program provider
