Am I eligible for case management?

​​CASC is a one-stop clinical reentry center that bridges probation supervision services with comprehensive support.

You must be clients of the San Francisco Adult Probation Department (SFAPD) to access our services. 

Contact your assigned Deputy Probation Officer for more details. 

Our providers

UCSF/Citywide

As the CASC's main service provider, Citywide provides both clinical and reentry case management services.

You will get:

  • A comprehensive assessment
  • Crisis intervention
  • Case management
  • Individual and group therapy
  • Substance abuse services

Senior Ex-Offender Program (SEOP)

SEOP provides case management services to clients who are 35 years of age or older. 

You will work with a primary case manager to identify needs, and will receive assistance in addressing barriers to your reentry. 

These services include:

  • Connecting you with public assistance
  • Medical services
  • Housing services 

M3 Case Management

M3 provides case management services to pretrial clients referred by the courts, as well as APD clients who are participants in Intensive Supervision Court (ISC).

You will be assessed, complete an individualized services plan and are enrolled in CASC programming with a special focus on substance dependency and recovery.

Other services include:

  • Housing planning
  • Shelter referral
  • Services brokering

Behavioral Health Services  

Adult Probation Department and the Department of Public Health partner to engage clients in behavioral health services.

Our services include:

  • One on one clinical therapy
  • Care coordination into substance dependency
  • Mental health treatment
