What to do
Go to the Department of Public Health, Office of Vital Records, room 105, 101 Grove St, San Francisco, CA 94102
Apply to get a medical marijuana ID card in person:
- Download and complete marijuana identification card application.
- Bring original letter from a medical provider stating that you have a medical condition where the use of medical marijuana is appropriate.
- Bring proof of identity (California Driver's License or ID Card, or Passport).
- Proof that you live in San Francisco (Utility bill, Photo ID, proof of recent rental or mortgage payment). A P.O. Box address is not acceptable for residency verification.
Caregivers:
- If you wish to designate a caregiver, you must designate on the application form.
- A primary caregiver must have a pre-existing relationship with the patient prior to enrolling in the program, including assisting the patient with housing, health and or safety.
- A patient can only designate one caregiver.
- The caregiver must be at least 18 years old.
- Your caretaker must accompany you when you apply. The fee for a primary caregiver is the same as the patient’s fee.
You can pay by Visa, MasterCard, or cash. You can also pay by money order, in US dollars.
