Go to the Department of Public Health, Office of Vital Records, room 105
Apply to get a medical marijuana ID in person:

Download and complete Medical Marijuana Application

Bring original letter from a medical provider stating that you have a medical condition where the use of medical marijuana is appropriate.

Bring proof of identity (California Driver's License or ID Card, or Passport)

Proof that you live in San Francisco (Utility bill, Photo ID, proof of recent rental or mortgage payment)
Caregivers:

If you wish to designate a caregiver you must designate on the application form.

A primary caregiver must have a pre-existing relationship with the patient prior to enrolling in the program, including assisting the patient with housing, health and or safety.

A patient can only designate one caregiver.

The caregiver must be at least 18 years old.
You can pay by Visa, MasterCard, or cash. You can also pay by money order, in US dollars.
