Complete your FBN Statement form.

The information you include on your FBN Statement must match the information you included to register your business with the County Clerk’s Office.

2. File with the County Clerk’s Office

You can file in-person or by mail. 

Be sure to check how you can pay your filing fee. It depends on if you are submitting in-person or by mail.

File your form in-person at the County Clerk's Office

Bring with you:

  • Your completed FBN Statement
  • The filing fee
  • A government photo ID, such as a driver’s license, state ID, or passport
  • A copy of your Business Registration Certificate or Temporary Verification of Registration (TVR) issued by SF Treasurer & Tax is required for new accounts and any changes to the following since your last FBN filing: DBA, business location, or ownership.
  • Original wet signature is required.
