Referral ng LTBI (Partikular sa mga Civil Surgeon)

Noong Oktubre 1, 2018, ang bagong 2018 Civil Surgeon TB Technical Instructions ay nagsasaad na ang pagsusuri sa IGRA ay kinakailangan para sa lahat ng mga aplikante na na-screen para sa pagsasaayos ng katayuan sa mga legal na permanenteng residente sa US na may edad na 2 taon o mas matanda at ngayon ay kinakailangan na mag-ulat ng LTBI sa departamento ng kalusugan. Kinakailangan ang chest X-ray para sa lahat ng aplikante na may positibong resulta ng IGRA, kilalang impeksyon sa human immunodeficiency virus (HIV), o mga palatandaan/sintomas ng sakit na TB. Hindi dapat i-refer ng mga civil surgeon ang mga aplikante sa departamento ng kalusugan para sa pagsusuri sa IGRA o chest X-ray; lahat ng IGRA at chest x-ray na iniutos ng mga sibilyang surgeon ay dapat isagawa nang hiwalay sa isang departamento ng kalusugan. Ang mga aplikanteng nangangailangan ng pagsusuri para sa aktibong sakit na TB (abnormal na chest X-ray o HIV infected) ay dapat na patuloy na iulat sa TB Control.

Dapat kumpletuhin ng mga civil surgeon ang Confidential Morbidity Report para iulat ang LTBI sa SFDPH TB Prevention and Control Program at i-fax ito sa (628) 206-4565. Para sa mga aplikanteng nakatira sa labas ng San Francisco City at County, dapat mag-ulat ang civil surgeon sa county ng paninirahan ng aplikante. Sa ngayon, hindi namin ginagamit ang CalREDIE Provider Portal.

The applicant's name, contact information, IGRA results, chest x-ray, and treatment results must be reported to the San Francisco Department of Public Health (SFDPH) Tuberculosis Control Program using the Confidential Morbidity Report Form Fax to (628) 206-4565.

Civil surgeons should offer LTBI treatment or refer patients to primary care for LTBI treatment.

All HIV-infected
applicants

History of HIV infection

If the applicant has known HIV infection, an IGRA and a chest x-ray must be performed and they must be referred to the health department of jurisdiction for sputum testing (regardless of above results).

The patient will be contacted to schedule an appointment to come to the clinic once the following is faxed:
- Civil Surgeon Checklist
- Confidential Morbidity Report
- Chest x-ray results
- IGRA results

Please ID yourself as a referring Civil Surgeon Provider and fax the documents to (628) 206-4565.

Active TB disease
(Suspected or
confirmed)

Abnormal Chest X-ray (including chest X-rays)
May or may not have signs / symptoms of active TB disease

Any confirmed or suspected case of active TB disease is required by law to be reported within one business day to the TB Prevention and Control Section by telephone at (628) 206-8524, or by fax at (628) 206-4565. This will also act as a referral and we will work with you to set up an appointment.

The patient will be contacted to schedule an appointment to come to the clinic once the following is faxed:
- Civil Surgeon Checklist
- Confidential Morbidity Report
- Chest X-ray results
- IGRA results
