How to file a whistleblower report

Report fraud, waste, and abuse by City employees and people who do business with the City.

1. Learn about the program

Before you file a report, learn about the Whistleblower Program.

Be sure to review:

  • How it works
  • Your rights (including confidentiality and protection from retaliation)

2. Submit your report

On your report form, tell us:

  • Your contact info (unless you would like to remain anonymous).
  • Full and complete complaint details, explaining who, what, where, when and why.
  • If you have reported this information to another venue.

Include any documents (photos, copies of emails) to support your report.

Tell the operator that you wish to file a report with the Controller's Whistleblower Program.

Check your report status

After you file a report, you'll get a complaint tracking number.

  • Use your tracking number to check your report status here for reports filed before
    July 1, 2024
  • Use your tracking number to check your report status here for reports filed on or
    after July 1, 2024.

When you check your report status, you will:

  • Get a general update on your report status
  • See any investigator messages if they have follow-up questions

Under current law, we do not disclose Whistleblower report information.
