Keep in mind
- You have 60 days from signing to cancel
- You must file a different form, called a Recission Form (DCSS 0915)
- This form must be signed by an authorized witness, like a Notary Public
- You must mail this form to the Parentage Opportunity Program
- You must send a copy of this form to the other parent, with proof of mailing
- You can't cancel if there's a court order related to your case

If either parent was under 18 when they signed the Voluntary Declaration of Parentage (VDOP) form, you might have more time to complete these steps. Usually, you have 60 days from the day you turn 18. Contact us for help understanding your deadline to cancel.
