What to do
1. Review the program guidelines
Program partners must follow program guidelines on vaccine use, storage and handling, documentation and reporting.
2. Follow instructions to enroll before the deadline
New partners: email our program at immunization@sfdph.org to confirm eligibility and request enrollment instructions.
Partners who participated in the 2023-24 program: follow re-enrollment instructions which will be sent to vaccine coordinators via email in July.
The deadline to enroll for all partners participating in the 2024-25 program is August 23, 2024.
