If you cannot find the record you want in your MyChart patient portal, you can:

Ask for the records in MyChart:

  1. Log in to MyChart
  2. Go to Menu
  3. Select Sharing Hub
  4. Select Yourself
  5. Select Request a copy

Or follow the steps listed on this page to ask for your records by mail, fax, or in person.
