You can view most of your medical records in your MyChart patient portal.

1. Download and fill out the following form:
- English version: Health release form
- Spanish version: Health release form
- Chinese version: Health release form
2. Bring the completed form and a valid photo ID to:

2. Find your clinic address on the last page of the form.
3. Mail your form to that clinic.
We will get back to you within 5 to 10 business days.
To pick up your records, bring a valid form of photo ID.

If you cannot find the record you want in your MyChart patient portal, you can:

- Log in to MyChart
- Go to Menu
- Select Sharing Hub
- Select Yourself
- Select Request a copy
