Pediatric symptom checklist (PSC) forms and information

Emotional and physical health go together in children. Because parents are often the first to notice a problem with their child’s behavior, emotions or learning, you may help your child get the best care possible by answering these questions. Also available in: Spanish, Chinese (traditional), Chinese (simplified), Vietnamese, Tagalog, and Russian. You can also download forms in other languages.

The Pediatric Symptom Checklist (PSC) is a 35-item parent/caregiver-report psychosocial screen designed to facilitate the recognition of cognitive, emotional, and behavioral problems so that appropriate interventions can be initiated as early as possible.

This is a report (automated in Avatar) that was created to provide written and visualized feedback to caregivers or parents, and it summarizes the overall needs of their children as well as those needs specific to four domains: attention, internalizing challenges, externalizing challenges, and school concerns.

This is an OPTIONAL tool that a care provider/clinician can use to give meaning to the PSC ratings.
