File an ADA complaint

Follow the steps to file a complaint about access to City facilities and services.

What to do

Tell us what happened

We will need to know:

  • Your name
  • Your address
  • Your phone number
  • The location of the incident
  • Date and time of the incident
  • Description of what happened

How to file your complaint

You should file a complaint in writing. If you have a disability, you can file a complaint by phone, or in other format that works for you.

Fill out as much of the online form as you can. Provide your contact information so we can follow up with you.

What's next

After we receive your complaint, we will provide the appropriate department a copy of your complaint. An ADA Coordinator will then conduct an investigation.

You will get a written response from us within 30 days after you filed your complaint.

Request reconsideration

Request reconsideration

You can request reconsideration if you are not satisfied with the resolution.

The reconsideration process:

1. Send the request for reconsideration to us in writing, or in an alternative format, if you have a disability.

2. Make sure you do this within 10 business days from the day the City issues a response to your complaint.

3. We will inform the departmental ADA Coordinator of your reconsideration request.

4. The Coordinator will respond to your request within 15 business days of receiving the request.

5. We must approve that response before we send it back to you.
