The Sunnydale Community Wellness Center is one of four clinics that provides behavioral health care, nursing services to assist with health care needs, and wellness programs to support the well-being of people living in Hope SF Communities (Alice Griffith, Hunters View, Sunnydale, Potrero Hill).
Hope SF clinics are operated by the Community Wellness Program, a partnership between SFDPH and Urban Services YMCA.
Services include:
- Health education and promotion literacy
- Assessments
- Physical activity classes
- Medication support
- Wellness activities
- Community events
- Support groups
- Case management (assessing needs, coordinating care, and connecting to other services)
- Individual and group therapy
- Crisis services
- Referrals to primary care
- Chronic disease and acute illness management
- Preventative and sexual healthcare
- Older adult care
- Persons living with disabilities care
- COVID19 testing
