  • Upload several photos that clearly show proof of significant damages to your business due to winter storm flooding. Include photos from different angles. We will reach out to you if we need additional photos or other documentation, such as a bid from a contractor to explain the extent of the damages.

  • Complete all applicable sections and sign the W9. The name you put on line 1 of the W9 form MUST MATCH and be identical to your tax return.
  • You can download and fill out a blank version of the W9 form.
