Get information about your cannabis business owners

The cannabis permit application asks for personal information and conviction history of all owners.

You will enter information about your business's owners when you apply for a permit. We will review every owner.

An owner can be:

  • a person or entity with an ownership interest of 20% or more of your business
  • the chief executive officer
  • members of the board of directors of a nonprofit
  • any person or entity that influences how your business will be run

Personal information

For every owner, you need to have:

  • Home address
  • Date and place of birth
  • Title in the cannabis business (like owner or CEO)
  • Percentage of ownership in the cannabis business
  • Name of current employer
  • If they have had a commercial cannabis license taken away in the last 3 years
  • Demographic data such as race, ethnicity, gender, income level, education (not mandatory)

Conviction history

In the application, you must tell us about serious, violent, drug trafficking, or fraud related convictions that any owners have. See the entire list of relevant convictions under Section 1615 (e) Discretionary Grounds for Denial.

When you get a background check through the FBI for the state application, you may discover out of state convictions.

If any owners have relevant convictions, we would like their:

  • Conviction date
  • Incarceration start and end dates
  • Probation start and end dates
  • Description of the offense, including the statute the conviction is under

These convictions could otherwise disqualify the business from getting a permit. But owners can prove that they are responsible enough to run a cannabis business.

You can upload a statement each owner has written for each conviction. Explain why you should get a cannabis business permit.

If available, you can upload a Certificate of Rehabilitation.

You can also upload reference letters from employers, instructors, or professional counselors. Date the letter and include your reference's contact information.
