What to do

1. Call to schedule an enrollment appointment

Call the Patient Access Enrollment Office to schedule a telephone appointment to apply for health care coverage program with a Certified Enrollment Worker. Our business hours are Monday - Friday, 8am-12pm and 1-5pm.

2. Documents needed during your enrollment appointment:

You will be required to provide the following documentation:

1. Picture Identification
2. Proof of Residency
3. Proof of Household Income
4. Proof of Household Assets

This documentation helps the Certified Enrollment Worker determine if you are eligible for an available program.  Programs may have different document requirements.

  1. Picture Identification – For example: Driver’s License, Consular ID, Permanent Resident or Temporary card, Passport or City ID
  2. Proof of Residency – For example: Most recent Rental Agreement, Utility Bill, Tax Bill, or Bank Statement
  3. Household Income – All that apply: Most Recent Pay stubs, Tax return, Award Letters: Unemployment, Disability, Social Security, Retirement
  4. Household Assets – All that apply:  Most Recent Bank statements (checking/savings), Bankbooks, Financial statements for Brokerage/Retirement Accounts

Additional documents may be required by the Enrollment Worker during the interview.  Your application will be considered incomplete until all documents are submitted.
