Over 15 outreach teams work on the streets every day. Teams help with medical care, behavioral health, case management, and housing. They can help people in the moment. They also build relationships and make connections with resources. Street teams have thousands of interactions with people each week.
Medical Outreach
Medical Outreach teams bring healthcare and services to people on the streets. They support connections to resources that help people stay healthy.

Behavioral Health Outreach
Behavioral Health teams work throughout the city. They assess need, make connections to resources, and provide ongoing engagement and follow-up.

Teams include: Behavioral Health Homeless Team, Glide’s Women’s Center and Harm Reduction Team, Felton’s Street Case Management, Engagement Specialist Team, and STARR Case Management, Richmond Area Multi-Services, Community Forward, and Episcopal Community Services.
Homeless Outreach Team
The San Francisco Homeless Outreach Team (SFHOT) has small teams that work with people experiencing homelessness on the street. They make connections and offer help. Teams help people get services, shelter and housing.
