Get a medical marijuana card

Apply to get a medical marijuana ID in person:

• Download and complete Medical Marijuana Application 

• Bring original letter from a medical provider stating that you have a medical condition where the use of medical marijuana is appropriate.

• Bring proof of identity (California Driver's License or ID Card, or Passport)

• Proof that you live in San Francisco (Utility bill, Photo ID, proof of recent rental or mortgage payment)

Caregivers:

• If you wish to designate a caregiver you must designate on the application form.

• A primary caregiver must have a pre-existing relationship with the patient prior to enrolling in the program, including assisting the patient with housing, health and or safety.

• A patient can only designate one caregiver.

• The caregiver must be at least 18 years old.

You can pay by Visa, MasterCard, or cash. You can also pay by money order, in US dollars.
