Measure Description
The average daily population represents the average number of residents occupying a bed at Laguna Honda Hospital on a daily basis.
Laguna Honda Hospital (LHH) is a skilled nursing and rehabilitation facility. It provides a range of services to San Franciscans who are disabled and chronically ill, including specialized care for those with wounds, head trauma, stroke, spinal cord and orthopedic injury, HIV/AIDS, and dementia.
Please note: LHH paused new admissions from March 2022 through July 2024 due to a recertification process with the Centers for Medicare and Medicaid Services (CMS). New targets for this measure are in development.

How Performance is Measured
The number displayed on the scorecard page represents a fiscal year average of the values in the chart above.
Laguna Honda Hospital is part of the San Francisco Health Network (SFHN). SFHN is a comprehensive system of care, overseen by the Department of Public Health (DPH), which includes primary and specialty care, dentistry, emergency and trauma treatment, skilled nursing and rehabilitation, and behavioral health. SFHN includes Laguna Honda Hospital, Zuckerberg San Francisco General Hospital, and numerous primary care clinics.
As of 2019, Laguna Honda Hospital included 780 skilled nursing and acute level of care beds. Patients categorized as skilled nursing rehab patients are patients receiving rehabilitative services for the effects of illnesses and injuries such as stroke, spinal cord injury, brain injury, and other major traumas. Acute patients generally have a shorter length of stay and require more rehabilitative sessions than skilled nursing patients.
