How Performance is Measured
The City of San Francisco operates programs to help households exit homelessness to housing. These programs provide permanent solutions to homelessness through permanent supportive housing, rapid rehousing, and problem solving.
- Permanent Supportive Housing places households into long-term affordable housing with a range of supportive services, including case management and housing retention assistance.
- Rapid Rehousing is a time-limited subsidy for tenants to live in private-market units and access supportive services.
- Problem Solving includes relocation assistance, financial assistance, and other interventions to divert or rapidly exit people from homelessness.

Data lag time: One month
Homeless service providers report data on clients served to the Department of Homelessness and Supportive Housing (HSH) on an ongoing basis. These programs were previously administered through the Human Services Agency and Department of Public Health but consolidated under HSH upon its formation in 2017.
The number displayed on the scorecard page represents a fiscal year-to-date sum of households who have moved into housing via permanent supportive housing and rapid rehousing, as well as households who achieved a successful problem-solving resolution.
Problem solving includes housing location assistance, travel and relocation support, mediation, and flexible financial assistance. Through FY21, reported problem solving numbers only included Homeward Bound. Homeward Bound provided money for transportation to stable housing with friends or family and sunset at the end of FY22. Relocation assistance is still offered through general Problem Solving programs.
Past data may be periodically updated due to additional data cleaning or methodology updates.
