If the incident is in progress or requires immediate emergency response, call 911
1. Fill out a form
Describe the incident involving the autonomous vehicle. Include as many details as you can including:
- When and where the incident occurred
- What and who was involved. For example, other vehicles or pedestrians.
- What company the autonomous vehicle belongs to. For example, Waymo or Zoox. If unknown, include a vehicle description or license plate number.
Including detailed time, time, location and any photos or videos facilitates review of incidents.
2. Track your case
After you report, you will get a tracking number from 311. You can use this number, and your email address if provided, to track your case online.
