Where to get your COVID-19 vaccine in SF. Healthcare workers and caregivers are eligible if they live or work in SF.

Vaccination is now available for all healthcare workers (HCWs) and caregivers.

There are different ways to access the vaccine, depending on your workplace.

Hospitals

Vaccination is provided by your employer.

These facilities include:

  • Skilled nursing facilities
  • Long-term care facilities 
  • Assisted living facilities
  • Residential care facilities for the elderly  

Vaccination is provided on-site at work by the Federal Pharmacy Partnership with Walgreens and CVS, or through alternate arrangements made by your employer.

Home health, home care, and in-home supportive services

Check first with your usual healthcare provider. Many have agreed to vaccinate their patients who are eligible.

If you provide home care or personal care services for someone, download this attestation form (translated form coming soon). The person you are caring for (or their signatory) needs to fill it out and sign it. Bring the completed form to your vaccination appointment. It proves you are eligible to get vaccinated.

If you work through a Regional Center, use the form provided by your Regional Center to prove you are eligible.

Outpatient healthcare and dental care facilities

You have several options:

  • For office and clinics affiliated with a hospital: Check with your hospital to see if it’s offering vaccine for affiliated outpatient healthcare workers.
  • Check with your usual healthcare provider. Many have agreed to vaccinate their patients who are healthcare workers.
