Nurse Home Visiting Programs (Field Public Health Nursing and Nurse Family Partnership)

 

 

You want the healthiest start for your baby… Let us help you get there! If you are pregnant or have a baby, a public health nurse is available to visit and provide you and your family with support, information and resources. All services are voluntary and free.

What we do

Your public health nurse meets with you to discuss your questions or concerns, and helps guide you to:

    • take steps for a healthy pregnancy
    • prepare for your baby
    • become a confident parent and help your baby learn and grow
    • support your family’s health
    • help your family connect to community resources
    • reach your goals and dreams in school, work, and life
    • get needed supplies (baby crib, diapers, etc.)

Who is eligible for Nurse Home Visiting Programs?

Pregnant and postpartum women who live in San Francisco are eligible for one of our two programs. We encourage moms to enroll as early in pregnancy as possible to get an early start on a healthy pregnancy and healthy baby.


Nurse Family Partnership Program is for women who are pregnant with their first child, in their first or second trimester (less than 28 weeks), and meets income requirements.


The Field Public Health Nursing Program is for all pregnant or postpartum women living in San Francisco, including those who already have children.


Our nurses speak multiple languages and can use interpreter services.


Nurse Home Visiting Programs are committed to provide quality health care and services for all San Franciscans, regardless of immigration or insurance status.

What can you expect from a public health nurse home visit?

Your public health nurse meets with you at your home or at an agreed upon safe place on a regular basis.  Other family members are welcome to participate in the visits as well. Your nurse will get to know you and help you have a healthy pregnancy and healthy baby.

Your nurse may…

    • talk with you about getting prenatal care, and preparing for labor
    • talk with you about how to take care of your newborn, and share positive parenting tips
    • check your blood pressure
    • help you with breastfeeding
    • weigh your baby
    • check your baby’s development
    • help you manage your day to day stress
