You can pay the application fee with a check or money order to the SF Department of Public Health. Mail or deliver the payment in-person to:

If sending by mail, include "ATTN: Well Program" in the address line.
3. Submit your items
Gather your items and send them to us:
- For monitoring wells, email your items to Eurich.Santiago@sfdph.org
- For production wells, email your items to Corey.Chrisman@sfdph.org
You can also mail or deliver your items in-person to:

4. Submit a report
- Make sure you are following state regulations
- Submit a copy of the Department of Water Resources Well Drillers Report, Form 188 with the State portal
- Email it to Eurich Santiago at Eurich.Santiago@sfdph.org within 30 days
Next steps
After you submit your application:
- The inspector will review your application and your annular seal
- The Tax Collector will bill you for the annual permit fees
