Approved companies

Businesses using these approved companies must also check photo ID:

• Android phone 
• Apple iOS 
• Bindle 
• CommonPass 
• CrowdPass 
• MyChart by Epic
  • Generates a QR code in your account.
• VaccineCheck 
• VXPASS
  • Must use VXPASS app/verification webpage and photo ID

Approved companies with ID verification included (businesses do not need to check):

• CLEAR HealthPass
• VaxYes
  • Use level 2 verification.

To add a company to the list

Submit a request to healthplan@sfcityatty.org to add a company to the approved list.  

In the request, say

• How the company verifies vaccine records
• How they confirm identity
• How they maintain privacy

Also, describe the accessibility features of the company. These include cost, languages, and platforms (Android, iOS, etc).

How to verify vaccine status

• Check name matches photo ID
• Check vaccine information or scan QR code with the SMART health care verifier app.
