Register to vote Use this link to register to vote, or re-register to update your name, address, or party preference.

Update your voter registration record Use this form to update your contact information and how you receive election materials. Note: you cannot use this form to change your name or party preference.

Request cancellation of your voter registration Use this form to cancel your voter registration in San Francisco. This form must be signed (electronic signature not accepted).

Third-party voter registration notification Use this form to notify the Department if a voter is receiving mail at an address where they no longer reside.

Voter oath and ballot return form Use this form if you are a military or overseas voter returning your ballot by fax, or if you are using your own envelope to return your ballot. 

Deceased voter notification Use this form to notify the Department of a deceased San Francisco voter.

Ballot pick-up authorization Use this form to authorize someone to pick up a ballot from the City Hall Voting Center and deliver it to you.

Emergency ballot service request Use this form to request emergency ballot delivery, pick up, or assistance from the Department of Elections. 

Federal Post Card Application Use this form to update your status a military or overseas voter. 

Federal Write-In Absentee Ballot Use if you're a military or overseas voter and are having trouble receiving your local ballot.
